Our House Calls team is directed by Dr. Ann Agnew, M.D.
Dr. Agnew has more than twenty five years experience in Internal Medicine. She has worked in hospitals as well as outpatient settings. A graduate of the University of Missouri Kansas City Honors Program in Medicine, she completed three years of internal medicine residency at Washington University Barnes Jewish Hospital. She has worked in Boise Idaho for 15 years.
Dr. Baker is an Board Certified Family Medicine Physician. She was born and raised in Idaho, attended University of Idaho for her undergrad, University of Utah School of Medicine and graduated from Family Medical Residency of Idaho (FMRI) in 2011. She has worked both inpatient and outpatient for Saint Alphonsus medical group since residency and has also developed a passion for house calls and hospice. Dr Baker is also an active member of the Idaho Medial Association and is the trustee to the IMA board of directors for the southwest medical society. She also sits in the U of U school of medicine admissions committee and works family Medicine faculty as the director for Idaho student programs. She is a proud mom is three and her hobbies include enjoying the Idaho outdoors with her husband and children.
Dr. Lazarte is a graduate from Weber State University in Ogden, Utah, where he obtained a bachelor degree in Microbiology and Organic Chemistry. He continued his education at Creighton University School of Medicine in Omaha, Nebraska, where he graduated with honors. He pursued an Internal Medicine residency at the same institution. Dr. Lazarte enjoys the challenge of taking care of chronically ill patients and believes that everyone deserves excellent care. For this reason, Dr. Lazarte founded Basis Internal Medicine where his dedication and commitment to excellence has been able to flourish. Dr. Lazarte lives in Eagle, Idaho and is actively involved in his community and enjoys spending time with his wife and children.
